A Life Care Plan is an organized, concise plan for current and future needs, with associated costs, for individuals who have experienced catastrophic injury or have chronic health conditions. By utilizing Life Care Planning services, the insurance carrier is better prepared to settle large claims in order to avoid long-term management costs. Eckman/Freeman employs case managers certified in Life Care Planning.

The Life Care Plan addresses:

  • Equipment needs, supplies, pharmaceuticals, therapeutic modalities, home health care or facility care, medical follow-up needs, and potential complications

  • Non-medical issues such as transportation and architectural renovations for home care

  • Educational/Vocational plan, if appropriate and requested

  • Services which will prevent complications and improve outcome
